Forms

There are a variety of ways to add a form to your site. It can be simple, like a contact form, or complex, such as a registration or application.

Plugin

Formidable Pro Forms plugin (external link) is a powerful plugin which allows you to easily create forms of all shapes and sizes – and so much more.

The Gravity Forms plugin (external link) is a full-featured form-building tool allowing you to create advanced forms.

Embed

You also have the ability to embed a Google Docs Form.

Theme

Some themes also have their own contact form templates and tools, depending on which you choose to use. Just remember that if you change your theme, you’ll then lose the form that is unique to that theme and will have to recreate it using your new theme or one of the methods above.

Quizzes & Polls

The Gravity Forms Quiz Add-On (external link) allows you to quickly create Quizzes that are automatically graded or scored when the form is submitted.
