Student Organization Recognition

The Student Organization Action Form lists the forms that you will need to submit in order to become a Penn State College of Medicine recognized Student Organization.

Please read through the Information Packet for Student Organizations for a more thorough explanation of how to establish a student organization at the Penn State College of Medicine.

As a Student Organization, you must also agree to abide by the 2018-2019 PSU Student Organization Policy and the 2018-2019 Student Organization Fund-raising Policy

Student Organization Action Form

This form lists the forms that are needed to 1) request recognition as a new organization 2) re-activate an organization that has been suspended or inactive or 3) notify MSSA & the Office of Student Affairs of an organizational leadership change.

Please send a copy of your Constitution and membership list to Amy Bockis and the HPSA.  A sample Constitution is included in the Information Packet.

We ask that every organization submit their End of Year Report by March 1st.  Forms are reviewed following HPSA’s final meeting of the year.  This form helps the HPSA budget for the upcoming year and ensures continuing recognition of your organization.  The aim in completing this form is to increase transparency, communication, and organization efforts here on campus.
