Many of you have told us that you are looking for simple ways to communicate your residents’ preferences to staff. One long term care community we’ve worked with developed this colorful and creative way to do just that! Attached is an example of a one-page “My Story” sheet that can be completed and hung inside a resident’s closet door or medicine cabinet.
Activity staff can help residents complete their story sheets during a group activity, or ask nursing assistants to complete them with the residents they care for. Consider treating staff to a breakfast, ice cream or pizza party once all the residents’ forms are completed! Feel free to use this as a template and use as is, or design your own “My Story” forms to suite your needs.
