Instructions:
There are several methods to retrieve your submitted questionnaires that have been returned and require edits:
- Method 1 – through email
- Method 2 – Searching for the document within the GA/SODI application
- Method 3 – Using the ‘My Questionnaires’ section of your Dashboard
Method 1: The first method is through Email
As the submitter, you will receive an email containing a link to the questionnaire form. Simply click on this link to access the questionnaire.
Method 2: The second method is to use the application’s Search utility.
From the application’s Home page, click the Search button.
You can use the ‘Text Search‘ option and input keywords.
Or, you can use the ‘Advanced Search‘ option to search by certain fields and/or by keywords.
Method 3: The final method is to use your Dashboards.
From the application’s Home page, click the Dashboards button. Scroll down to the ‘My Questionnaires’ section and click the button to access those Returned.
Then click the plus sign to expand your list and find and click the pencil icon for the relevant one to open it in edit mode.
Input Requested Information:
Based on the feedback you received from the Gift Planning Office, input the additional information requested and/or input information necessary for clarification. For example, suppose I was asked to provide more information regarding student eligibility. I would scroll down to the “Please describe the current population of eligible students” question and input the requested information.
Tip: Notice that there is a section displaying the questionnaire’s action history.
And when you’re ready, click Resubmit. Then click Ok to confirm.
You will receive a confirmation email.
What happens next?
- The resubmitted questionnaire is reviewed again by the Office of Gift Planning personnel and it then goes through an approval process (involving serveral offices).
- Then there is a “Finalization of Reviews” by the Gift Planning Office and the Guideline Agreement (GA) or the Statement of Donor Intent (SODI) is generated and then “Released” for signatures.
- The original form submitter receives an email containing a link to the Released document/pdf. At this point, unit personnel can submit a request for a new Allocation code and they are to verify that donors, gift officers, unit leadership, and any other persons who proof drafted documents have edited and approved the draft copies. Then they can request to have Gift Planning obtain e-signatures of the donor(s).
- Once the Office of Gift Planning (OGP) have obtained donor and all other signers’ signatures, OGP and other signers receive an email notice from Adobe Sign and copies of the fully signed documents will be attached to the email. OGP then “Execute” the fully signed GA/SODI. Once executed, the donor will receive via email a hard copy of their GA/SODI printed on “blue” endowment paper along with their acknowledgement letter signed by Rich Bundy. And if relevant, an Endowment Certificate.
- Finally, the Office of Gift Planning will scan the document to DocFinity for it to be accessible in iFiles.
